What Is Dry Needling?

Dry needling is a modern therapeutic technique that uses an extremely thin, sterile needle (similar to an acupuncture needle) to target “trigger points” in your muscles – those sensitive, tight knots that cause pain and limit movement. Unlike an injection, the needle is “dry” (no medication is injected); simply inserting it into a tense muscle spot causes a reflex that releases the knot and helps reset the muscle to a relaxed state. Many patients feel a twitch in the muscle when the needle hits the trigger point – that’s a good sign the tight spot is releasing! After a brief treatment, blood flow increases in the area and the muscle often relaxes, leading to less pain and better mobility.
In simpler terms, dry needling lets us reach deep muscle knots that our hands can’t quite get to with massage alone. By nudging those irritable spots, we “reset” the muscle and jump-start your body’s natural healing response. Painful, overactive muscles calm down, flexibility improves, and you can move more freely without that constant ache. Best of all, dry needling works fast – often patients notice relief immediately or within a session or two, which is a huge motivator to keep up with the rest of their therapy exercises.

How Is Dry Needling Different from Acupuncture?

At first glance, dry needling looks a lot like acupuncture – after all, both use very thin needles. However, the two practices are quite different in philosophy and technique. Acupuncture is an ancient Chinese medicine practice that places needles along specific energy pathways (meridians) to balance the body’s “qi” (energy flow). An acupuncturist might treat, say, back pain by inserting needles in seemingly unrelated points (like your ear or foot) based on those meridian maps. The needles often stay in for 10-20 minutes while you relax.

Dry needling, on the other hand, is rooted in modern Western medicine. We insert the needle directly into the tight muscle or trigger point that’s causing your pain. There’s no mysticism – it’s a straightforward physical approach to release knots and reset muscle function. Typically, far fewer needles are used in dry needling (often just a few in the affected area), and they are inserted for only a brief time (a few seconds up to a couple minutes) to get that twitch response and release, then removed. In short, acupuncture treats pain by balancing energy along meridians, whereas dry needling treats pain by targeting the exact muscle knots causing trouble. Both can be effective for pain, but dry needling is usually quicker to produce relief in musculoskeletal issues and is most often used as one component of a physical therapy plan.

How does it feel?

Despite the scary sound of “needles,” patients usually handle dry needling very well. The needles are hair-thin – you barely feel them break the skin. You might feel a tiny pinch or a muscle cramp sensation for a second, especially if we hit a sensitive trigger point (that twitch we mentioned). Afterwards, the area may feel sore for a day (like a good workout soreness), but many people also report an immediate sense of relief and looser movement. We always explain everything beforehand and make sure you’re comfortable – if you’re ever uneasy about it, just let us know. Conditions Dry Needling Can Help

Dry needling can be a game-changer for a variety of painful conditions – particularly those involving muscular tightness, trigger points, or overuse. It’s not magic or a stand-alone cure-all, but as part of a well-rounded treatment plan it can provide rapid relief from stubborn pain and help you move forward with healing. We often combine dry needling with targeted exercises, hands-on manual therapy, and movement retraining. By addressing both the symptoms (muscle knots, tension) and the root causes (like weak stabilizing muscles or poor movement patterns), we set you up for lasting results.  Dry Needling for Knee Pain (and IT Band Syndrome)

Knee pain is incredibly common, especially among active adults in Chattanooga who run, squat, hike, and stay on the move. Often, knee pain isn’t just about the knee joint – the muscles above and below (quads, hamstrings, calves, hip muscles) play a huge role. If those muscles are tight or knotted up, they tug on the knee and can cause persistent pain. Dry needling helps by releasing that muscle tension and restoring normal muscle function around the knee.
 
One common culprit in knee issues is IT Band Syndrome, which causes pain along the outer knee. The IT band itself is a thick fascia, but the tension often comes from trigger points in the tensor fascia lata (TFL) muscle near the hip and the vastus lateralis (part of the quad) along the thigh. Dry needling those muscles can relieve the tension on the IT band. Essentially, we release the muscular knots contributing to the friction and inflammation at the knee. Of course, we pair this with correcting any underlying issues (like weak hip stabilizers or foot mechanics) so the relief is long-lasting. But the immediate pain reduction from dry needling often allows you to rehab more comfortably.

Dry needling is also great for post-surgical knee pain or stiffness. If you’ve had something like an ACL reconstruction or knee scope, you might have residual tight spots in the quads, hamstrings, or calf that make it hard to move normally. By carefully needling those areas (always ensuring your surgical sites are well-healed and it’s safe to do so), we can break the pain-spasm cycle. It helps increase your knee bend and decrease pain, complementing the stretching and strengthening we do in therapy.

Real Client Success Story: From Knee Pain to Pain-Free Squats

One of my clients came in with persistent knee pain – the kind that flares up during squats and just wouldn’t let her move freely. She had a history of ACL surgery and was struggling with pain on the front of her knee anytime she loaded that leg.

We tested her single leg squat at the start of the session and she rated her pain a 3–4 out of 10. After needling her quadriceps and hamstrings – areas often overlooked post-ACL – something clicked.

She was able to do multiple single leg squats pain-free.
